Want to make a suggestion to the practice?

Church Lane Medical Centre strives to provide excellent patient experience and a range of high quality services.

In areas you feel we are achieving this we appreciate your compliments. In areas you feel we can improve we also welcome your suggestions and feedback . This can be done via our suggestions box, patient surveys or our form.

Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

Want to make a complaint?

Every patient has the right to make a complaint about the treatment or care they have received at Church Lane Medical Centre.

We understand that we may not always get everything right and, by telling us about the problem you have encountered, we will be able to improve our services and patient experience.

A complaint to the practice can be made either verbally or in writing.  A complaints form is available from reception or you can complete the online form on our website.  Alternatively you can complain via email to nl.b81064@nhs.net.

The Complaints Managers for the practice are Kirstie Hornsey (Deputy Practice Manager) and Faith Woodcock (Practice/Business Manager).  The Responsible Person for Complaints is Dr T Hussain (Senior Partner).
